The annual disclosure certification period begins March 15, 2022 and disclosure forms are due by April 15, 2022.

Those who are required to complete a disclosure form will receive an email notification from MyDisclosures between March 15 and 18. Do not complete a disclosure form before March 15, 2022, because they will not satisfy the annual requirement. 

Starting in March, information regarding training sessions for disclosers, supervisors who review disclosure forms and administrators will be available here.
